Help! I Have Sciatica — Or Do I?

Does Every Disc Bulge Cause Sciatica?

Pain shooting down your leg? Don’t assume the scan has all the answers.

If you’ve had an MRI and been told you have a disc bulge, it’s easy to assume:

“That must be what’s causing my pain.”

But that’s not always the case.

A disc bulge can contribute to sciatica, but plenty of people have disc bulges with absolutely no symptoms.

And just because pain travels into your buttock or leg doesn’t automatically mean you have true sciatica either.

That distinction matters — because the right treatment depends on knowing what’s actually driving your symptoms.

What Is a Disc Bulge?

Between each vertebra in your spine sits an intervertebral disc.

These discs help:

  • Absorb load
  • Allow your spine to bend and twist
  • Distribute forces through the lower back

A disc bulge happens when part of the disc extends beyond its usual boundary.

This can develop gradually over time or occur after an injury or period of heavy loading.

But here’s the important bit:

A disc bulge does not automatically mean something is badly damaged.

Disc changes are incredibly common, especially as we get older.

Many people have them without ever experiencing back pain or sciatica.

What Is True Sciatica?

Sciatica is often used as a catch-all term for any pain travelling down the leg.

But true sciatica specifically involves irritation of a spinal nerve root that contributes to the sciatic nerve.

When that nerve becomes irritated, symptoms can travel from the lower back or buttock into the leg.

Common signs include:

  • Sharp, burning or electric pain
  • Pain travelling below the knee
  • Pins and needles
  • Numbness
  • Weakness in the leg or foot
  • Symptoms aggravated by coughing, sneezing or straining
  • Pain that worsens with certain sitting or bending positions

People often describe it as more of a shooting, burning or electric sensation than a muscular ache.

What Isn’t Sciatica?

Not every pain that spreads into the buttock or thigh is nerve pain.

Other structures can refer pain into the leg, including:

  • Muscles
  • Spinal joints
  • Sacroiliac joints
  • Ligaments
  • Tendons

This is called referred pain.

Compared with true sciatica, referred pain is more likely to:

  • Stay above the knee
  • Feel dull or aching
  • Not cause pins and needles
  • Not cause numbness
  • Not cause muscle weakness

That’s why treating all leg pain as “sciatica” can send rehab in completely the wrong direction.

Can a Disc Bulge Cause Sciatica?

Yes — but only if the disc is actually affecting a nearby nerve.

This might happen if the disc:

  • Irritates a nerve root
  • Causes inflammation around the nerve
  • Reduces the space available for the nerve

When this occurs, symptoms tend to follow a recognisable nerve pattern into the leg.

But if the disc bulge isn’t affecting a nerve?

It may be completely pain-free.

The MRI Doesn’t Tell the Whole Story

This is one of the most important things to understand.

Seeing words like:

  • Disc bulge
  • Degeneration
  • Disc protrusion
  • Wear and tear

on an MRI report can sound scary.

But spinal changes become increasingly common with age, even in people with no pain.

Think of them a bit like wrinkles on the skin.

They can be normal changes that come with time — not proof of damage.

That’s why we don’t treat the scan.

We treat the person.

At Fighting Fit, we combine any imaging findings with:

  • Your symptoms
  • Your history
  • Neurological testing
  • Strength testing
  • Movement assessment
  • Functional testing

That gives us a much clearer picture of what’s actually going on.

Can You Have Sciatica Without a Disc Bulge?

Absolutely.

Disc problems are only one potential cause.

Other causes can include:

  • Spinal stenosis
  • Degenerative changes around the nerve
  • Spondylolisthesis
  • Inflammation around a nerve root
  • Deep gluteal syndrome

And, much less commonly, more serious conditions can also irritate nerves.

That’s why a proper assessment matters.

Signs Your Symptoms May Be Disc-Related

Some patterns can make disc involvement more likely, including:

  • Pain beginning after bending, lifting or twisting
  • Leg pain being worse than back pain
  • Pain travelling below the knee
  • Pain with coughing, sneezing or straining
  • Difficulty sitting for long periods
  • Symptoms changing with repeated movements or positions

These signs can help guide diagnosis, but none of them prove the cause on their own.

Do You Need an MRI?

Usually, not straight away.

Most cases of back pain and sciatica can be managed initially without imaging.

An MRI may be more appropriate if:

  • Symptoms aren’t improving with appropriate treatment
  • Significant weakness is developing
  • Symptoms are getting progressively worse
  • A serious condition is suspected
  • Surgery or injection treatment is being considered

Your physio or GP can help decide whether imaging is actually needed.

Can Physiotherapy Help Sciatica?

Yes.

For most people, physiotherapy is a first-line treatment.

Treatment depends on the cause, but may include:

  • Education about your condition
  • Exercises to improve mobility
  • Nerve gliding exercises
  • Progressive strengthening
  • Advice on staying active
  • Load management
  • Manual therapy where appropriate
  • A graded return to work, training or sport

The goal isn’t just to settle the pain.

It’s to restore strength, confidence and movement while reducing irritation of the nerve.

When Should You Seek Urgent Medical Attention?

Most cases of sciatica are not emergencies.

But seek urgent medical assessment if you develop:

  • Loss of bladder or bowel control
  • Numbness around the groin or saddle area
  • Rapidly worsening leg weakness
  • Difficulty walking due to progressive weakness
  • Severe pain after significant trauma
  • New back pain with fever, unexplained weight loss or a history of cancer

These symptoms require prompt medical investigation.

The Bottom Line

A disc bulge does not automatically mean you have sciatica.

And pain travelling down your leg does not automatically mean the sciatic nerve is involved.

The most important thing is identifying the true source of your symptoms.

Because:

  • Treating a scan won’t always fix the pain.
  • Stretching a leg won’t fix nerve irritation.
  • Rest alone rarely solves the problem.

A proper assessment gives you clarity — and a plan that actually matches what’s going on.

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.

This is the second time I have been treated by the team at FFP.
