Steady Steps Forward: Tackling Chronic Ankle Instability with Modern Techniques

Have you sprained your ankle so many times that you’ve lost count? 

If so, you’re not alone!

You should know that it isn’t just another ankle sprain, it is probably chronic ankle instability (CAI). 

This condition can make your ankle feel wobbly, leading to repeat injuries and even long-term issues like arthritis. 

But don’t worry—there’s good news!

New therapies, like electrical stimulation, are offering fresh hope to people with CAI.

Let’s break down what CAI is, how it affects your body, and how new thinking is leading to old treatments like neuromuscular electrical stimulation (NMES) and transcutaneous electrical nerve stimulation (TENS) being used effectively to help you get back on your feet—literally.

What Is Chronic Ankle Instability?

CAI often develops after repeated ankle sprains. Over time, those injuries can weaken the stabilizing muscles and mess with your brain’s ability to control the joint. 

This makes it easier to sprain your ankle again—and again—and again.

But here’s the catch: the problem isn’t just in your ankle. CAI affects how your brain and nervous system communicate with the muscles that stabilize and move your joint. This means the issue runs deeper than just strength, mobility or flexibility.

Meet NMES and TENS: Our High-Tech Helpers

These tools use electrical currents to stimulate your muscles and nerves, offering a little extra “push” to get things back on track.

NMES (Neuromuscular Electrical Stimulation): Think of this as a workout for your ankle muscles. NMES sends stronger electrical pulses then your brain can following injury that cause your muscles to contract to its full potential. This helps them actually get stronger and more responsive quicker than swinging your ankle with band or balancing on something unsteady.

TENS (Transcutaneous Electrical Nerve Stimulation): This option is gentler, focusing more on stimulating nerves than causing muscle contractions. It’s often used for pain relief, but recent studies show it may also improve how your brain and nerves work together. This can be really useful when immobilised, pain limits movement or trauma causes an inability to use the joint or surrounding muscles normally. 

What Does the Research Say?

A recent study looked at how NMES and TENS might help people with CAI. The results are a mixed bag—but still exciting!

TENS Showed Promise: People who used TENS had improved nerve responses in their ankle, meaning their nervous system became better at communicating with their muscles. However, this didn’t yet translate to big improvements in balance or strength.

NMES Didn’t Shine as Bright: Surprisingly, NMES didn’t lead to significant changes in the study. This could be because the treatment parameters weren’t quite right or because ankle muscles respond differently to this therapy compared to other muscles, like those around the knee.

 

The Big Takeaway:

Neither tool was a quick fix on its own, but TENS and NMES can have potential for “rewiring” the nervous system. To make a real difference in how your ankle functions, these treatments might need to be paired with exercises.

What do we say clinically??

Often what research reports and what we see in clinical practice can be different as clinical trials are very pure and unbiased, unlike real life. 

We have found that using NMES and in some cases TENS can assist early on in delivering a stimulus to the muscles and nerves around the ankle to promote activation. This is best used when the body cannot do it itself, whether delayed in firing due to CAI or even during injury healing and early recovery from the first ankle sprain. 

How Can This Help You?

Teamwork Makes the Dream Work: Electrical stimulation alone isn’t a cure, but it can be a great partner to rehab exercises. 

 

Think of it as jump-starting your progress, firing up a dormant muscle or nervous system that isn’t or hasn’t been working optimally for some time. NMES can boost the signal when the brain’s message isn’t getting there, retaining the delivery electrical impulses to the stabilise the ankle once again. 



Personalized Plans Are Key: Everyone’s body is different. Your physiotherapist can help decide whether NMES, TENS, or both are right for you based on your specific needs. 

Be Patient and Persistent: Results might not be instant. Building ankle strength and retraining your nervous system takes time, but the long-term benefits are worth it.

Stay Involved: Ask your physiotherapist questions and learn about your treatment. Understanding how these tools work can keep you motivated and confident in your recovery.

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.

This is the second time I have been treated by the team at FFP.
