A recent study surveyed 143 physiotherapists to see how they manage rotator cuff-related shoulder pain.
The results were encouraging.
The overwhelming majority based their treatment on current evidence, with exercise forming the foundation of rehabilitation.
That’s exactly what modern clinical guidelines recommend.
Exercise Is Still the Best Treatment
Almost every physiotherapist in the study prescribed exercise as the primary treatment.

- Isometric exercises
- Upper limb strengthening

Many patients achieve excellent long-term outcomes through physiotherapy alone.
Where Does Hands-On Treatment Fit?
Manual therapy can absolutely play a role.
Joint mobilisations and soft tissue techniques may help reduce pain and improve movement—particularly early on.
But they work best when they’re combined with exercise.
Passive treatment might help you feel better today.
