Getting Back on Your Feet: The keys to a successful ACL recovery

If you’ve ever known someone who’s undergone anterior cruciate ligament (ACL) reconstruction, or if you’ve experienced it yourself, you’ll know it’s a tough journey. 

But with the right rehabilitation plan, recovery isn’t just about regaining movement—it’s about coming back stronger. 

Let’s share some cutting-edge insights to help you understand the best path to recovery after ACL surgery.

Why Is ACL Rehab So Important?

The ACL is a critical ligament in your knee, stabilizing it during dynamic activities like running, jumping, and pivoting. Injuries to the ACL can be devastating, with long recovery times and, unfortunately, a high risk of reinjury. Research shows that only 65% of people return to their pre-injury level of sport, and up to one-third of young athletes re-injure their knee within two years of returning to play.

What’s the solution? 

High-quality, structured rehabilitation that starts from day one—even before surgery. 

Let’s break it down into six key areas that can set the foundation for success.

  1. Managing Pain and Swelling post ACLR – quickly & effectively

Pain and swelling are common after ACL reconstruction and can limit your ability to move and regain muscle strength. 

Early on, techniques like icing, compression, elevation, and gentle range-of-motion exercises are crucial. 

Swelling can and will restrict knee movement and impair muscle activation, making it essential to address early. Regular assessments, such as measuring knee circumference and tracking pain levels, can help monitor progress and adjust treatment plans.

Cryotherapy (icing) is particularly effective when used before strength exercises, as research shows it can reduce muscle inhibition and improve quadriceps activation. This proactive approach lays the groundwork for faster and more effective recovery.

  1. Restoring Joint Range of Motion

Getting your knee to fully straighten and bend is vital. Full extension (straightening) is especially important for walking normally and preventing complications like scar tissue formation (arthrofibrosis).

Loss of extension within the first three weeks post-surgery is a strong predictor of long-term issues, including pain and stiffness.

To regain range of motion, early-stage exercises often include:

Passive stretches to improve extension.

Gentle flexion exercises, such as stationary cycling or wall slides.

Hydrotherapy, which uses water’s buoyancy to facilitate safer movement.

Achieving at least 120 degrees of knee flexion by six weeks post-surgery allows patients to transition to more advanced activities like cycling and eventually running.

  1. Building Muscle Strength

Did you know your quadriceps—the muscles at the front of your thigh—can lose up to 50% of their strength in the first month after surgery? 

That’s why rebuilding muscle is a top priority. Quadriceps weakness is associated with impaired knee function, altered gait, and a higher risk of reinjury. Restoring lower limb strength symmetry as quickly as possible increases chances of positive outcomes and reduce re-injury risk!

Early strategies we use to rebuild muscle strength include:

Isometric exercises: Holding static positions to activate muscles without joint movement.

Neuromuscular electrical stimulation (NMES): Using electrical currents to stimulate muscle contractions, particularly useful when pain or swelling limits voluntary movement.

Blood flow restriction (BFR) training: Performing low-load exercises with a cuff to reduce blood flow, which enhances strength and muscle growth safely during early stages.

Progression involves moving from low-load, high-repetition exercises to more dynamic and functional activities as the knee heals.

 

  1. Improving Movement Quality

Learning how to move properly is just as important as getting stronger. 

Poor movement patterns, like favoring one leg, can increase the risk of reinjury. 

Early rehab focuses on balance, coordination, and control, starting with simple activities like:

Gait retraining: Ensuring proper walking mechanics, often using video analysis.

Bilateral squats with visual feedback: Practicing symmetrical loading with live feedback will help to avoid ongoing movement compensation.  

Pool-based exercises: Leveraging the reduced impact of water to work on movement patterns.

Research highlights that early movement quality improvements (e.g., during walking or basic squats) predict better performance in complex tasks like running and jumping later on.

  1. Supporting Your Mental and Emotional Health

Recovering from an ACL injury isn’t just a physical journey; it’s an emotional one too. 

Many patients experience frustration, anxiety, or even depression. Building a trusting relationship with your physiotherapist and accessing emotional support can make a big difference.

Key psychological strategies include:

Setting short- and long-term goals to maintain motivation.

Engaging in mindfulness or relaxation techniques to reduce anxiety.

Seeking peer support from others who’ve undergone similar injuries.

  1. Staying Fit While You Heal

Even though your knee is out of action, it doesn’t mean the rest of your body has to be. 

Maintaining cardiovascular fitness and strengthening other muscle groups (like your core and hips) can keep you feeling strong and ready for the next stages of rehab.

Examples include:

Upper-body ergometry: Using hand bikes to maintain aerobic fitness.

Cross-training: Working the uninjured leg to promote a phenomenon called cross-education, where strength improvements in one leg carry over to the other.

Core and hip exercises: Strengthening these areas improves overall movement quality and reduces strain on the knee.

The Bigger Picture: A Full Recovery

Successful ACL rehab isn’t about rushing back to sports; it’s about doing it right. 

That means:

Setting a clear plan and goals for each stage of recovery.

Using objective criteria, like strength tests and movement assessments, to decide when to progress.

Focusing on injury prevention strategies to reduce the risk of reinjury.

Taking the time and not rushing the outcome. ACL rehabilitation takes 12 months, done well you will return to sport. Done poorly, it will take longer and success rates drop!

Whether you’re an elite athlete or someone who just wants to get back to daily life, the principles of rehab are the same. 

Early and comprehensive rehabilitation improves not only knee function but also long-term quality of life.

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.

This is the second time I have been treated by the team at FFP.
