Is Your Kneecap Out of Whack?

 Here’s What You Need to Know About Patellar Malalignment!

Does your knee feel dodgy when you squat, climb stairs, or sit for too long?

That sharp or achy pain around the kneecap might not be a simple case of overuse. It could be patellar malalignment — when your kneecap isn’t tracking where it should within the groove of your thigh bone. Left untreated, it can mess with your movement, cause constant discomfort, and stop you doing the things you love.

At Fighting Fit Physiotherapy, we take this condition seriously — because it’s not just about pain. It’s about performance, prevention, and long-term results.

 

What is Patellar Malalignment?

Patellar malalignment happens when your kneecap (patella) shifts out of its ideal position in the femoral groove. This misalignment often links to soft tissue imbalances or structural quirks in the way your hips, knees, and legs work together.

It commonly shows up alongside:

  • Patellofemoral pain syndrome
  • Osgood-Schlatter disease
  • Patellar tendinopathy (aka jumper’s knee)

And yep — it’s more than just a local knee issue. It’s a chain-reaction kind of problem.

How It Presents: The Telltale Signs

Here’s what we often see in clinic:

  • A high-riding or low-riding kneecap (patella alta/baja)
  • A wider Q-angle (which affects alignment)
  • Tight outer thigh tissues — especially the IT band and lateral retinaculum
  • Weak hip muscles, especially external rotators
  • Pain with stairs, squats, sitting too long, or getting up from a chair

 

Inside the knee, the tug-of-war between your quads matters. If the vastus medialis obliquus (VMO) isn’t pulling its weight, the patella tends to drift sideways — often resulting in that all-too-familiar click or sharp twinge.



How We Diagnose It at Fighting Fit

There’s no one-size-fits-all test. Instead, our physios take a full-body view:

✅ Gait and posture analysis
✅ Knee alignment and VMO activity checks
✅ Movement assessments (e.g. squats, stairs)
✅ Hands-on palpation and patellar mobility tests

Imaging may help in more complex cases, but we usually find the answers in your story and how you move.

 

How We Fix It: Proven Physio Strategies That Work

Here’s our multi-pronged plan to get your kneecap tracking right and your life back on track:

🔹 1. Fire Up the Right Muscles

We target your VMO and hip stability system with:

  • Mini squats with bands
  • Ball wall squats (extra points for a ball between the knees)
  • Side-lying leg lifts and glute bridges
  • Straight leg raises with a VMO-friendly twist

🔹 2. Stretch What’s Tight

Tension in the wrong spots pulls your kneecap sideways. We loosen up with:

  • IT band rolling
  • Hip flexor and quad stretches
  • Patella mobilisations
  • Myofascial release along the outer thigh

🔹 3. Sharpen Proprioception

You need more than strength — your body needs to know where your knee is in space.
We use:

  • Bosu balance drills
  • Single-leg stability training
  • Functional movement retraining

🔹 4. Tape It Right

We’re fans of the McConnell taping technique (yep, invented by Aussie physio Jenny McConnell in the ‘80s). It’s precise and powerful. We also use bracing or kinesiology taping when needed to guide movement and ease pain while you build strength.

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.

This is the second time I have been treated by the team at FFP.
