Why Ankles Get Stiff After Injury

Ankle injuries are incredibly common especially in runners, active adults, and motocross riders. Whether it’s a simple rolled ankle or something more complex like a high ankle sprain, one of the most frustrating symptoms that lingers is stiffness. Many people expect the pain to settle quickly but are surprised when the ankle still feels tight, blocked, or “just not right” weeks later.

So why does ankle stiffness occur after an injury, and how does physiotherapy help restore movement?

  1. Swelling and Joint Effusion

When you sprain or injure your ankle, your body sends fluid to the area as part of the inflammatory response. This swelling increases pressure inside the joint and around the soft tissues, reducing movement. Even mild swelling can limit ankle movement. This process is an important part of healing to help initially protect the ankle from further damage and to kick start the healing process. But if you continue to have swelling weeks to months down the track, that can cause issues.

  1. Protective Muscle Guarding

Following an injury, the muscles around the ankle can switch into protective mode. They tighten up to prevent further damage. While helpful in the first few days, this muscle guarding becomes counterproductive if it lingers, restricting normal motion.

  1. Ligament Tightness and Scar Tissue Formation

As ligaments heal, they lay down scar tissue. Early on, this scar tissue is disorganised and sticky. Without guided movement, it can cause the joint to feel restricted or stiff. 

  1. Capsular Tightness

The ankle joint capsule can stiffen if movement is limited after an injury. Think of it like a tight, shrinking envelope around the joint. Without early mobility, the capsule can become a major contributor to long-term stiffness. This is why putting a ankle in a bot to immobilise it can actually cause more harm them good without the proper management.

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.
