Understanding and Managing Hip Labral Tears in Soccer: How Physiotherapy Can Help You Recover and Prevent Further Injury

But now her hip hurts!

Why? 

Hip labral tears are a common and often debilitating injury that can affect soccer players of all levels. A study done by Márquez et al. (2018) evaluated 42 professional soccer players’ hips with 3T magnetic resonance imaging. They found 33.8% had a labral lesion, 12.5%  of the players had a partial tear, and 2.5% had a complete tear.1 

These injuries not only cause significant discomfort but can also severely limit your performance on the field. Whether you’re a professional athlete or a weekend warrior, understanding the causes, symptoms, and treatment options for hip labral tears is essential for a successful recovery and continued participation in soccer. 

In this post, we will explore the anatomy of the hip labrum, common causes of labral tears in soccer players, and how physiotherapy can play a vital role in both treating and preventing these injuries.

What is a Labral Tear in the hip?

The hip labrum is a ring of cartilage that surrounds the acetabulum (the socket of the hip joint). 

Its main function is to stabilize the hip and provide smooth, friction-free movement during activities such as running, pivoting, and kicking. 

In soccer, where rapid changes in direction, high-impact collisions, and repetitive movements are common, the hip labrum is at risk of being injured. A tear in the labrum can occur due to trauma, repetitive motion, or structural abnormalities in the hip joint.

 

Causes and Symptoms of Hip Labral Tears in Soccer Players

Causes:
Soccer players are particularly susceptible to hip labral tears due to the intense nature of the sport. Common causes include:

  • Trauma: Trauma from tackles, falls, or direct contact during a game can cause acute labral tears. Additionally, the repetitive nature of certain movements in soccer (like twisting, kicking, and sprinting) can gradually wear down the labrum.
  • Hip Impingement: In soccer, players are often required to rotate their hips quickly, which can lead to impingement. This occurs when abnormal contact between the femoral head (the ball of the hip joint) and the acetabulum increases stress on the labrum, eventually leading to a tear.
  • Overuse and Repetitive Motion: Kicking, especially with intense force, repeatedly stresses the hip joint and can contribute to labral tears over time.

Symptoms: The symptoms of a hip labral tear can vary but often include:

  • Pain in the groin, outer hip, or front of the hip, especially after intense activity.
  • Clicking, locking, or catching sensations in the hip joint.
  • Limited range of motion, making movements like kicking or twisting difficult.
  • Pain aggravated by activities like long-distance running, kicking the ball, or specific movements like twisting or turning.

Physiotherapy Solutions for Hip Labral Tears in Soccer Players

While some hip labral tears may require surgery, many soccer players can find relief and return to play through physiotherapy. One of the skilled physiotherapists here at Fighting Fit  will assess the severity of your injury and develop a personalized rehabilitation program. 

This program will typically include a combination of strengthening exercises, stretching, manual therapy & joint mobilization, and pain management strategies.

1. Strengthening Exercises for labral tears

Building strength in the muscles surrounding the hip joint is essential for relieving stress on the labrum and enhancing overall stability. Key muscle groups to focus on include:

  • Hip Abductors: Strengthening the muscles on the outside of the hip helps stabilize the joint during high-impact movements like running and turning.
  • Glutes: Exercises like glute bridges and clamshells target the gluteus medius and minimus, muscles critical for hip control and preventing abnormal hip movement during soccer play.
  • Core Strengthening: A strong core is key to maintaining pelvic stability and reducing strain on the hip joint, especially during explosive movements like sprinting or jumping.

2. Stretching and Flexibility of the hip

Soccer players rely on flexibility for quick movements and agility. Gentle stretching can improve hip flexibility and reduce stiffness, especially in key areas like the hip flexors, hamstrings, and adductors. Stiffness in these muscles can exacerbate the symptoms of a labral tear and limit movement during the game.

  • Hip Flexor Stretches: These stretches help lengthen the muscles at the front of the hip, improving mobility.
  • Hamstring and Quadriceps Stretches: These stretches can improve flexibility in the posterior and anterior muscles, enhancing your kicking range and stride length.
  • Piriformis and Glute Stretches: Relieving tension in the deep hip muscles can improve overall function and reduce discomfort during lateral movements and quick pivots.

3. Manual Therapy & Joint Mobilization Techniques for Hip Pain

Manual therapy, including joint mobilization, can help alleviate stiffness and improve hip mobility. 

A physiotherapist may use these techniques to restore normal movement patterns in the hip joint and reduce pain, which is especially important for movements like turning, kicking, and jumping.



4. Activity Modification and Ergonomics for Hip Pain

A physiotherapist will help you modify your training and playing routine to prevent aggravating your hip injury. This might involve:

  • Avoiding high-impact activities or adjusting your training load.
  • Switching to lower-impact activities such as swimming or cycling to maintain fitness while the hip heals.
  • Improving posture and movement patterns to reduce stress on the hip during specific soccer movements.

5. Hip Pain Management Strategies

Managing pain and inflammation is critical for a successful recovery. Modalities such as:

  • Ice and Heat Therapy: These can help reduce inflammation and relieve acute discomfort following activity.
  • Medication: simple analgesia like anti-inflammatories and paracetamol (consult with a GP or pharmacist on use)
  • Manual Therapy: is a great intervention for hip pain sufferes to ease pain, reduce muscle tone and improve movement of the hip. 
  • Exercise: yep, specific exercises are super useful to manage pain and improve function. They are a must!

Be AWARE they’ll immediately try to commit you to multiple follow up sessions after your initial consultation. This happened to me and then I later found out that as it just so happens, they have a cancellation policy that says you have to pay 100% of the consultation fee for cancelling. They failed to tell me this when they were pushy in committing me to 6 sessions after my consult, so that was a really nice little surprise.

I was already uncomfortable with how pushy they were in having me commit to so many sessions, so when I heard about the policy I just cancelled all together. So, I can’t speak further to the quality of their service throughout therapy phase. FYI this was also a few months ago now, it’s possible (though unlikely) they may have changed the policy. Just ask the question.

This is the second time I have been treated by the team at FFP.
